The forms you print must be clear and sharp. Applications that are blurred
or illegible will not be accepted. The form must be printed on white paper
with black ink or toner. The paper must be letter size (8.5 x 11 inches), with
no holes or perforations. Thermal paper, dye-sublimation paper, special
inkjet paper, and other shiny papers are not acceptable. |

Affidavit Of Identifying Witness | ||
![]() The Affidavit of Identifying Witness (Form DS-71) is used by the U.S. Department of State for individuals applying for a U.S. passport. This form is completed only by the identifying witness and serves to confirm the identity of the passport applicant. Essentially, it is required when the applicant for a passport cannot establish their identity to the satisfaction of a person authorized to accept passport applications. The applicant must still present some identification of their own. The identifying witness must have known the applicant for at least 2 years, be a U.S. citizen or permanent resident, and fill out the form in the presence of a Passport Agent. |
